How to obtain the certificate and where

Residents should apply to the Rejstrík Trestu GP CR, in person or by mail at Soudni 1, 145 00 Praha 4
Non-residents should apply in person at the nearest Czech diplomatic mission
Forms available in Prague at Rejstrik Trestu or any Registra's office (matrika) or District Prosecutor's office (Statni zastupitelstvi) in town of residence

 
Documents needed
